Sometimes you can have some additional information in your prescription (ADD) which is related to reading glasses. Please fill this in as well if you have it, although it is not mandatory so don't worry if it's not on your prescription.

 
Please be careful with plus and minus; Minus means that you are short sighted (myopic) - (your prescription form may also have a dash over the number). Plus means that you are long sighted (hyperopic).
